Name: Aurora Username: jamesmpf Rank before kick/leave: Sergeant with DDi clearance, archive site can confirm the rank How long have you been in sapa: A few months Who kicked you: HQ collaboration Date of kick/leave: Unknown Reason for kick/leave: Behavior/Attitude issues What happened from your side/why did you leave? : I was highly disrespectful and wasn't really mature during my employment, so I was removed. Why do you want back in? : To go about what I used to do which was assisting those who were in need. What have you learned from this? : To keep my attitude and behavior in check, venting in other ways instead of via a keyboard. Who do you want to apologize to? : The entire department as an entity; to the cadets I was assisting for not being able to train you properly with my behavior issues. And to the entire Instructor team for not representing the department as expected. Why should we give you a second chance?: To show that I've changed and with my knowledge I could be a good asset for the department. As a prior instructor and experience from other areas (such as SAI, SAFP and FOX) I could put that knowledge to scenarios during teaching.

When did you join the server: Roughly 2 years ago, I don't recall the actual date. How much do you play each day: Roughly 5 hours but it depends on the work hours I get within my part-time job. Previous Squads/Gangs/Companies: San Andreas ProCop Academy, San Andreas Federal Police, The Strike Team and FOX. Name the reason(s) for leaving: SAPA; Resigned after gaining my diploma to head for another squad. FOX; Resigned to head for a more traffic enforcement/law enforcement role play environment. TST; Kicked due to failing to give an inactivity notice due to massive personal issues at the time. SAFP; Kicked due to arguing with a staff member. (Appealed) 1 temporary ban for arguing too much with others, the ban was basically a way to tell me it's time to cool off. 1 mute for harassing. III. Personal skills and knowledge (*= this question has to be answered in full sentences) Your strengths:* I would say that my strengths include vehicle operations, advanced role-play and team-work. Your weaknesses:* My weaknesses include my ongoing temper/issues with arguing and firearms usage. Why do you want to join S.W.A.T.*: I'm seeking to join the SWAT Team so that hopefully they can kick me where it hurts so I can build up maturity and discipline. Furthermore, I'm looking to join a squad that focuses on team-work and builds their members up to become better in the future. Why do you think you will be good for S.W.A.T.*: I have the role-play knowledge from SAI, the ability to work well in a team and learn from my mistakes. Yes my temper does get out of hand sometimes but I feel as that can be controlled, using that negativity to boost production in other areas. Describe a SWAT specific RP and how you would approach the chosen situation (Requirements: minimum 150 words): Role-play type; Bank Robbery As the shift was coming to a close for the SWAT Team, a hot call came in regarding an ongoing armed robbery at the Los Santos National Bank. Each member would be divided into teams to maintain proper integrity and get the best possible outcome from the situation. Each team would consist of two to three officers, depending on the on-duty officers at the time. Team One and Team Two would engage the situation by entering through the main entrance on ground level while Team Three consisted of an air support unit to watch the roof for any subjects or suspicious activity. For support on the mission, a Tactical Paramedic would assist ground units by providing medical attention along with the San Andreas Medics if available. Ground teams would respond via Enforcers and Tahoe's (SUV's) while the air unit would respond via a Police Maverick. Before heading out each team would load up their respective vehicles with M4's for assault, beanbag shotguns for less-lethal take-downs, flash-bang and smoke grenades for tactical support as well as a sniper rifle for support via the air unit. Furthermore, the tactical paramedic would load their ambulance with medical equipment, ranging from tourniquets to medical kits and gauze. Furthermore; a call would be placed to the San Andreas Medics for support and local police to secure the perimeter around the bank. As the SWAT Team arrived at the scene, the air unit would relay any activity on the roof as the ground teams went to locate the local police who may have information in regards to the bank layout. Team One would the Enforcer as sort of a mobile command center, attempting to gain access to the bank security feed to get visual inside. Once visual is gained; ground units would stack on the main doors and prepare to breach if they can confirm no civilians are in the lobby, leading to casualties or injuries. A: If the lobby is clear, the breach would continue as normal. B: If the lobby holds hostile subjects with hostages, negotiations would take place before a tactical breach and support from Sierra 1 (sniper in the air unit). Moving on; Once the lobby has been secured by the SWAT Team, they would continue to sweep the floors until reaching the vault. Upon sweeping, the beanbag shotgun in combination with brute force in numbers would take down the subjects who are willing to comply. For those who aren't going to comply, the M4 would be used. We now reach the vault, where the main suspects are. The layout of the vault has two branching paths that both lead down into the vault, so Team One would take one path and Team Two the other. On a set time, both teams would flash and smoke the subjects while using their force to either shoot them down (if they don't comply and act hostile) or stun them with the shotgun to detain and furthermore arrest them. For the subjects who are complying, they are led out of the bank and to local police for transportation to the county jail. Air Unit; During the time of the initial breach, Team Three would be monitoring the roof and the progress via bank windows. If any suspect was to try and escape via the roof, the air unit would hover by the roof and give verbal commands as Sierra 1 trained their sights on them. If the subjects refused to cooperate and kept trying to escape; Team Three would radio the location and maintain visual on them as local police intervened. If they try to open fire at the unit, Sierra 1 would take them down immediately. Tactical Paramedic; During the entire time, the tactical paramedic would maintain support outside to the other officers as a second paramedic went with the ground team. If any officer was to be injured during the situation, the paramedic would transfer them to the closest hospital for treatment (or treat on the scene, depends on the injury). After the bank is secured and confirmed to be clear, the SWAT Team would exit and relay their findings to the local police on scene. LSPD and other units would take care of the scene and re-opening the location as the SWAT Team returned to base for debriefing.
  5. Section A: Information Nickname: Aurora Account name: jamesmpf Age: 21 Primary Language: Italian Current S/G/C and rank: San Andrea's Federal Police <Federal Officer> Other RP groups you're in: Black Eagle Security <Deputy Director> Describe SAM in your own words: (Not less than 30 words) The San Andrea's Medics is a team of highly knowledge and educated emergency personnel who specialize in a variety of areas, including: AirEvac (Air Transport), emergency response, basic and advanced treatment of various injuries and medical issues, triage control, etc...Their main priority is to provide to notch medical service and increased role-play within the emergency services field. Why was SAM founded? Due to a high lack of medical supplies being transported efficiently, the low response time of medical staff to scenes, poor logistical management and unusable vehicles; SAM was created and founded to stop the corruption and poor medical issues as listed. What is the SAM motto? Always there when you need us most. Who founded SAM? Doctor (Dr.) Joe Valachi Section B: Theory A major earthquake has occurred in metropolitan Los Santos and there are many casualties. You're based in Blueberry and must respond to a barrage of emergency distress calls as a result of the earthquake. Describe your method of response to this situation and be sure to include how you would interact with your fellow medics and which equipment & vehicles you would use. X: When I receive the call in Blueberry, I would record information on; Location, rough estimate on the number of victims, scene stability, and victim triage. After all information is recorded, I'd dispatch at least two AirEvac, four ambulances, and at least four fire engines to different locations of Los Santos. Within this, I'd have the local and state police departments secure all the areas before anyone arrives to ensure proper working conditions. Once the staff are at the scene I'd have the AirEvac scout the area and make sure ambulances can get to the victims. Ambulances would then start to move towards the victim areas and prepare a safe-zone that is out of the aftershock and earthquake zone to ensure proper treatment can allocate. Fire Departments would begin to asses and secure those trapped in vehicles or at heights. Due to it being an earthquake, and high area for falls and broken bones, I'd have the medical staff ensure that all patients have a proper BPM and clear airways before treatment starts. Once the BPM and airways are secured the medical staff would check for broken bones, and if any secure the area with a temporary splint, C-Collar (if on the neck/spine), and lay them on a backboard/stretcher so that they remain flat within five minutes of treatment. If there are no broken bones, but instead a sprained area, I'd have it (injured area) wrapped in a bandages to ensure the area is within healing position. Moving on; If the head is in any way injured I'd have a C-Collar placed to secure the neck, and then proper bandaging and cleaning of the wounded area. If the head is fractured, I'd have them rushed to the closest hospital for surgery. In the case of a vehicle accident; I'd have the Fire Department secure the vehicles and the victim(s) so that medical can treat them properly. Once all patients are secured I would have them transported to the hospital due to a high possibility of trauma in each of the victims. Multiple officers have been shot while responding to an armed bank robbery. You have been called to assist in the rescue operation. Describe how you would respond. X: When I receive the call; I'd attempt to record information on; Number of victims, scene stability, and victim triage (how serious the wounds are). As for response; I'd send out at least three ambulances to the bank location and wait until the officers on scene state it's clear to enter. I'd also have at least one AirEvac on standby for those who have serious enough injuries. Once on scene, all medical staff would enter with officers to protect them while the medical staff enters with a trauma bag, backboard/stretcher, C-Collar, and medical kits. Once entered all staff would begin a triage, heading to the most sever wounds first to secure them. For each wound, I'd have the staff apply pressure to the wounded area and cleanse it with an antibiotic. Next; I'd have the wound wrapped tightly in a bandage and gauze, to secure the bleeding to a minimum scale. Once this is done, we'd secure the body for transport to the closest hospital as surgery is required, as in some cases the bullet is still within the body causing more internal damage. All victims will be transported to the hospital, no matter if the bullet only penetrated the epidermis of the skin or if the bullet went all the way through hitting no bones or vital organs or nerves. Multiple people have been stabbed and burned by several terrorists at a train station and you 're the first medic to arrive on scene. The wounds are of varying severity. What would you do? I.e. How would you treat the wounds? X: When I receive the call; I'd record information on; Number of victims, scene stability, and victim triage (How bad are the injuries). Once this is recorded and I'm on scene I'd call for more assistance, then going to any officer on scene to ensure the scene is secured enough for me to start treatment and transport of the victims. Once clearance is granted; I'd notice the high amount of victims and go to the burn victims, setting up buckets of ice water and bottled water to give them so that they can clean their burned areas while I go to the stabbed victims. Once I'm with the stabbed victims I'd secure the wounds by cleaning the area with a wet cloth, and then wrapping a tight bandage and gauze around the area while secure the area in proper transport such as; Splints for legs and arms, backboards and C-Collars, temporary casts, etc. I'd also ensure that the breathing and BPM are stable before they are moved and prepared for immediate transport as the item used to piece the skin could have been infected, causing internal issues. Once the victims are secured I'd move to the burned victims and start cleaning the area with cold water or antibiotic gel, depending on how bad the burn is; And then loosely wrap the area in gauze with antibiotic spray or gel, preparing them for transport IF the burn is 2nd degree or higher. If the burn is 1st degree, I don't need to worry about transporting them unless they have other injuries. Section C : Aptitudes How would you respond to criticism by a superior?; I feel as criticism is a good thing and I can use it to better myself in the future. Why exactly are you applying today?; I have always loved emergency role-play, whether in law enforcement or fire and medical. I'm applying for SAM so that I may increase my medical role-play knowledge and provide a high service to the server. What are your strengths and weaknesses? Strengths: Driving, role-play, and activity ||| Weaknesses; Anger issues and flying Why should we accept you to our team?; I feel as I could assist the team with my activity and knowledge of locations so that I can respond quickly to any sort of call. Even though I have failed in the past, I feel as I'm prepared for a new start and a new change in the field.

  10. Section A: Information Nickname: Aurora Account name: jamesmpf Age: 21 Primary Language: Italian Current S/G/C and rank: San Andreas Federal Police, Sergeant Other RP groups you're in: Black Eagle Security (Deputy Director) Describe SAM in your own words: (Not less than 30 words); The San Andreas Medics is a team of highly skilled and trained Paramedics/EMT's who dedicate their lives to protecting and healing the citizens of San Andreas. They are here to maintain activity in the medical field and provide the top-notch area of medical role-play. Why was SAM founded? SAM was founded to counter the lack of medical services around San Andreas, such as low response time (if any) and poor medical treatment. What is the SAM motto? Always there when you need us most. Who founded SAM? Dr. Joe Valachi Section B: Theory A major earthquake has occurred in metropolitan Los Santos and there are many casualties. You're based in Blueberry and must respond to a barrage of emergency distress calls as a result of the earthquake. Describe your method of response to this situation and be sure to include how you would interact with your fellow medics and which equipment & vehicles you would use. Base: At the base when receiving the calls, I'd start getting response teams organized. Vehicles Used: Medical Evacuation (MED-EVAC) Helicopter(s), Ambulances and SAFD Search and Rescue Interaction/Details: Once the call comes in, I'd immediately contact the SAFD Search and Rescue team to coordinate with the MED-EVAC Helicopter to get gurneys, ropes and search gear set up in case anyone has fallen into a hole opened by the earthquake. Following that, I'd contact the San Andreas Interceptors and local agencies to secure the metropolitan area, block the roads and re-direct traffic away for safety. Lastly, I'd send a minimum of two ambulances out to the scene (one on each side of the metropolitan area) and set up medical triage centers at each location with local medical teams and volunteer rescue teams. I'd head to the scene in a third ambulance and then contact all teams to head to a central location. I'd coordinate with the SAFD High Ranks and have them clear any buildings, searching for medical issues with a SAFD EMT. Next step is coordinating the teams to establish medical triage areas for those who aren't injured that much. Next, having the SAFD Search and Rescue team and SAM Teams moving to get any victims who are seriously hurt. Scenarios: If we see someone stuck under rubble, I'd contact SAFD to use a jack to raise the debre safely, at this time I'd also place an IV into their arm to get fluids to the body due to the lack of flow to the crushed area. I'd then move the subject to a gurney and stretcher, getting them into the ambulance and rushing to the nearest hospital. |||| If I see someone in a hole, I'd contact the MED-EVAC team and have them hover over the hole where they are, lowering a gurney on a rope to the hole with a paramedic on board. The paramedic will examine the damage, secure the suspect on the gurney, raising it and flying to the closest hospital if fall damage is present (such as broken limbs). Multiple officers have been shot while responding to an armed bank robbery. You have been called to assist in the rescue operation. Describe how you would respond. Base: At the base when receiving the calls, I'd start getting response teams organized. Vehicles Used: Medical Evacuation (MED-EVAC) and Ambulances Interaction/Details: Once the call comes in, I'd immediately coordinate with local medical teams and SAM teams to head to the bank, establishing a triage area across from the bank in a secure location. Once on scene, I'd coordinate with tactical teams and local police, sending SWAT Tactical Paramedics and SAM Medics into the bank area to secure any officers who are shot and outside the bank. At this time, I'd coordinate with officers on the inside of the bank to ensure the bank is secured, then sending paramedics/SAM into the bank to secure the victims. Scenarios: If the victim has gunshots in a non-vital area (legs, feet, hands and arms), we'd secure the gunshot by applying pressure to the wound, cleaning the area with antibiotics and a cloth rag. Once cleaned, I'd place a clean cloth rag on the wound applying pressure and then securing with gauze or medical tape, ensuring the rag doesn't move. If the victim has gunshots in a vital area, we'd secure the wound in the same way and also have oxygen flowing if they're unconscious (also doing CPR if they aren't breathing). The victims with vital injuries will be MED-EVACd to the hospital for surgery and those without life-threatning injuries will be taken by ambulance. Multiple people have been stabbed and burned by several terrorists at a train station and you 're the first medic to arrive on scene. The wounds are of varying severity. What would you do? I.e. How would you treat the wounds? Base: At the base when receiving the calls, I'd start getting response teams organized. Vehicles Used: Medical Evacuation (MED-EVAC) Helicopter(s), Ambulances and SWAT/FBI Teams Interaction/Details: Once the call comes in, I'd immediately contact the SWAT/FBI Teams to send them to the area, confirming that no other terrorists are in the area and no bombs are in the area and the scene is secured. I'd send out Ambulances to the area while keeping MED-EVACS on standby. Once on scene, I'd have a triage area set up, one for burn victims and one for stab victims. I'd also contact the rail station to ensure that the station is shut-down and no trains come through for safety. Once SWAT/FBI clears the area and secures it, SAM would commence in establishing victim status. Sending victims to either the burn or stab triage areas. Scenarios: If a victim is unconscious, I'd check the breathing (If they're breathing i'd check the airways and make sure that consistent air flow is going into the lungs) and if not breathing start CPR. They'd be rushed to hospital via ambulance. Victims with first and minimal second degree burns, cleanse the area with cold water, antibiotics and ensure the skin is secured with gauze and antibiotics to ensure that the spread of infection and viruses is low. Victims with high second and third degree burns, they'd be rushed to hospital after the area is cleaned and secured due to the possibility of nerve damage. Stab victims will be treated the same as gunshot victims, securing and cleaning the area and then transporting for surgery (all victims) to repair the skin and possibly damaged muscle and tissue. Section C : Aptitudes 1.How would you respond to criticism by a superior? I'd take it and use it to better my future career. 2.Why exactly are you applying today? I'm applying so that I can provide a high service to the medical field, and gain more role-play experience. 3.What are your strengths and weaknesses? Strengths; Driving, location knowledge and discipline || Weaknesses; Flying and multi-tasking 4.Why should we accept you to our team? Because I have the knowledge and know-how of this job. I'm looking to provide a high service and have been for a long time on SAES. From SAI to previous SAM employment, I know the ins and outs of the job and can preform exceptionally.
